Please read below for information regarding the company's United Healthcare medical insurance, Guardian dental/vision/life/STD insurance, and UHC pet insurance, available to all full-time/year-round associates.  Even if you declined all coverage, you will still receive $15,000 in complimentary life and AD&D and are eligible for Guardian perks.

UNDERSTAND YOUR UHC BENEFITS

  • Go to myuhc.com and register so that you can review your plan benefits and coverage, find in-network doctors and facilities, check estimated costs for common procedures/conditions, and view/sort/pay your out-of-pocket costs.  Download the UnitedHealthcare app to find care, share your digital ID card, and more.

  • Virtual care services include general medical, dermatology, mental health, back and neck care and is available to all your dependents, even those not currently on your medical coverage.  Download the HealthiestYou app, visit the website or call 1-866-703-1259 to set up your account for 24/7 virtual care.

  • You also have access to Expert Medical Services to review your case when dealing with a difficult diagnosis/treatment plan/surgery.  Call 1-866-904-0910 for Expert Medical Services. Virtual care costs vary by plan.

  • If you enrolled in the HP200022, the HP20003060, or the HE635021 medical plans, you qualify to make pre-tax contributions into a Health Savings Account (HSA). The annual limit on 2024 HSA contributions for self-only coverage is $4,150. For family coverage, the HSA contribution is $8,300. Participants 55 and older can contribute an extra $1,000 to their HSAs. If you have questions about Health Savings Accounts, please contact hr@humanresources.com.

UNDERSTAND YOUR GUARDIAN BENEFITS

  • Register at guardiananytime.com/registration and follow the directions outlined on the Register on Guardian Anytime pdf.  Once you establish a profile you will be able to view your Guardian ID number, view a summary of benefits, print ID cards and find cost estimates, submit claims and receive alerts when claims are paid.  For your reference, the Group number for Paloma Legacy, LLC is 520436.

  • Save money when you use a provider who is in-network.  For dentists, visit guardianlife.com/find-a-dentist. For eye doctors, visit: vsp.com/eye-doctor.  Both the Base and Buy-Up dental plans cover preventative care (cleaning every 6 months, fluoride, oral exam, sealant, and x-rays) at 100%.   • If you are enrolled in STD, claims can be submitted quickly via TeleGuard.  Click here for details.

  • As of January 2024, the elimination period for STD is now waived for new mothers.  Click here for details.  

  • As a Guardian enrollee, you receive additional benefits, including identify theft & resolution, will preparation services, legal & financial services, Perks at Work discounts, and an Employee Assistance Plan (EAP) which includes counseling, child and elder care referral, bill negotiation tools, online legal documentation prep, and more.  (Even if you only have the company's complimentary life insurance, you are still eligible for these Guardian add-ons.)
